    NICE guidance is all about supporting people to quit which is fine for those who want to. But what about people who don't want to quit -be they patients, visitors or carers? How will their choice be supported?     How much lower can the CQC stoop? “We would… like to have an open conversation with people about the use of mystery shoppers and hidden cameras, and whether they would contribute to promoting a culture of safety and quality, while respecting people’s rights to privacy and dignity. Can I tell you now. THEY WON'T! They'll do the exact opposite and promote a culture of fear where staff feel they cannot trust their instincts in case a spy or a camera misinterprets their actions. Our institutions are in crisis and the CQC, Monitor, Ofsted etc just add to their problems.     While this 'research' adds to the evidence on the health benefits of breastfeeding once could be led to believe that the evidence for the superiority of breastfeeding across the board is unassailable. But it’s not - as two new books on the subject, Suzanne Barston’s Bottled Up and Zoe Kleinman’s Birth, Boobs and Bad Advice, reveal. These authors are by no means the first to question the pro-breastfeeding campaign. Joan Wolf of the University of Texas shook up the debate when she published her earth-scorching critique, Is Breast Best?, in 2010, and many other distinguished academics, have challenged the consensus about breastfeeding. The difference is that Barston and Kleinman, both journalists, are writing as mothers and for mothers; mothers who feel they were sold a bill of goods when it comes to infant feeding.     While it's true that bad practice has always co-existed alongside good practice, I would contend that something very specific has happened to influence how we now care, making poor practice much more widespread.The origins (and therefore solutions) I suggest lie mainly in the social and political realms.     Small, individual, electronic devices for use at the patient's bedside that sends their data to a central point where it's continually updated would be a good start.....I can already hear the chorus of 'no you cant do that' because..... of the expense, health and safety, infection control, privacy, safeguarding etc. But why not?

    Forgive me for being sceptical but I'd take this 'research' finding with a fry-up, a pinch of salt and a large glass of wine! For a critical analysis of healthy eating, food and the obesity panic, I'd highly recommend Rob Lyon's revealing book, Panic on a Plate: How Society Developed an Eating Disorder. He draws on history, science and official reports to show that fear-mongers are wrong and assesses the changing face of food positively.

    Jacintha Saldanha's death is very sad indeed but I think it's only fair to ask whether if the culture of celebrity surrounding the Royal Family wasn't there, would such shame be attached to her passing the call to another member of staff who gave out the private information? This may seem a cruel thing to say, but ultimately it was Saldanha who was responsible for her death. I think it'd set a dangerous precedent if the DJs were prosecuted for something that was intended to be just irreverent fun however much it may not be to everyone's taste.

    In preparation for the discussion that I'm hosting on Sat 20th at the Battle of Ideas 'A crisis of Compassion: who cares? 'http://www.battleofideas.org.uk/index.php/2012/session_detail/6776 I've been trying to get to grips with Martha Nussbaum's 'Compassion and Terror'. She suggests that compassionate judgement necessitates that we understand our world as one in which bad things happen to people, and human beings are both dignified and needy. This complex interaction, and our own ability to recognise our personal encounters with future tragedies, is what inspires compassion. Food for thought....

      Howard is attempting to address a very real problem here in relation to nursing care but I’m not convinced that strengthening the recruitment process and/or using the NHS Constitution are the answers. That’s why I'm organising a discussion 'A crisis of compassion: who cares?' during the Battle of Ideas festival at London's Barbican Centre 20-21 Oct 12. There we’ll explore how we tackle this supposed compassion deficit? Is the problem institutional, the NHS too big and bureaucratic, the private sector too interested in profit-making to care? Or is there a deeper cultural problem? Do we as a society care less about each other? Does the crisis in compassion for the elderly reflect a deeper lack of respect for wisdom and experience?       This piece -Inside an abortion clinic http://www.guardian.co.uk/world/2011/sep/01/life-inside-an-abortion-clinic prompted the CE of BPAS to comment on FB ''this is why I think being bpas chief exec is the best job on the planet. How good is this clinic and these staff?!'
